3.4.6 Innovations in end of life care

Innovative approaches are transforming some aspects of end-of-life care, for example by making it more accessible, personalized, and culturally sensitive. One major shift is the move towards digital tools and telemedicine, which are breaking down geographical and logistical barriers. Modern online platforms and apps allow patients to report symptoms daily from their own devices, enabling nurses to monitor their condition remotely and prioritize home visits based on real-time need (InHealthcare 2025). This not only empowers patients to have more control over their care but also helps to prevent unnecessary hospital admissions.

Such technology is complemented by hospice-at-home models, which bring a multidisciplinary team directly to the patient’s residence. These teams can provide expert care, including symptom management and emotional support, in the comfort of a person’s home, aligning with the strong preference of most people to die at home (Hospice UK, 2025b). The integration of telemedicine, remote monitoring, and home-based care represents a significant evolution from traditional care models, ensuring continuous, coordinated support for the patient and their family.

Beyond technology, innovations also focus on creating more inclusive and culturally competent services. As populations become more diverse, there is a growing recognition that care must be tailored to different cultural, religious, and spiritual beliefs. Cultural competency training for healthcare professionals is a key part of this, moving beyond a checklist approach to foster “cultural humility,” where providers are encouraged to be open, curious, and willing to learn from their patients’ unique perspectives (HPC Consultation Services, 2024).
